Information needed to schedule a pathology consultation:

​​

•  The following will be needed for completing new consultation forms:

  •  Patient’s full name

  •  DOB 

  •  Copy of state-issued photo identification 

  •  Address/phone number/email

  •  Preference of communication 

  •  Reason for consultation (diagnosis)

  •  Referring physicians name (if applicable)

• Send your pathology report to PPC if you already have a copy along with a copy of your driver's license -or-

• Sign a consent release if you are not the patient 

  • **You will need to contact medical records where you were seen.  The records release can be emailed or faxed back to Personal Pathology Consulting, which is on our release form. ​
